The goal of this module is to educate physicians and other health-care providers about case management services in Texas, including the Case Management for Children and Pregnant Women program.

Specific Learning Objectives

After completing the activities of this module, you will be able to:

  1. Indicate two ways in which patients and families benefit from case management.
  2. Specify one way that health-care providers benefit from case management.
  3. Differentiate between the roles of the case management provider and the health-care provider.
  4. Specify one condition that could result in a high-risk pregnancy and qualify a woman for the Case Management for Children and Pregnant Women program.
  5. Interpret one situation in which a child with a health condition may need case management services.
  6. Recognize the role of case management as it applies to the Early Childhood Intervention (ECI) program.
